ASSESSMENT AND REFERRAL SERVICE

The Adult Evaluation and Review Service is a program designed to assist geriatric adults who are at risk of losing their independence.  A registered nurse or a social worker provides medical and psychological evaluations, consultations, referrals, linkage to community resources, and coordination of services.  These evaluations are performed in the clients home, hospital, or nursing facility.  The program also provides pre-admission screening for nursing home and long-term planning.
